Step One: Informed Consent
Before we meet, please read my Informed Consent Psychotherapy document, which will explain to you my professional qualifications, office practices, and agreement for services. This includes important information about confidentiality, the risks and benefits of psychotherapy, fees, missed appointments and cancellation policies, etc. If you have any questions about this document please do not hesitate to ask me.
If you or your child will be seeing Dr. Reist or an associate under her supervision, please read the Informed Consent Psychoeducational Assessment which will explain to you our professional qualifications, office practices, and agreement for services. This includes important information about confidentiality, the risks and benefits of an assessment, fees, missed appointment and cancellation policies, etc. We will go over informed consent when we meet and if you have any questions, please don’t hesitate to ask us.
Step Two: Print and Fill Forms
If possible, please print and fill out the following forms and bring them to your first appointment. This will enable us to get started right away. Forms will also be available in the waiting room. If you want to fill forms out in the waiting room, please come 15 minutes early to your first appointment.
