Before your first appointment, there are some documents you will need to read that will provide you with important information.

Informed consent is a required process to ensure that you are fully informed about your rights and responsibilities and that you give your consent to proceed. Please take a moment to read the document(s) below before your first appointment.

Informed Consent

If you or your child will be seeing me or an associate under my supervision, please read the Informed Consent for Psychoeducational Assessment,  which will explain to you my professional qualifications, office practices, and agreement for services. This includes important information about confidentiality, the risks and benefits of an assessment, fees, missed appointment and cancellation policies, etc.  We will go over informed consent when we meet and if you have any questions,  please don’t hesitate to ask me.

My Privacy Statement provides you with information about how I protect your personal health information.

Patient Forms

If possible, please print and fill out the following forms and bring them to your first appointment.  This will enable us to get started right away.  Forms will also be available in the waiting room.  Please come 10 minutes early to your first appointment.
